HOW IT WORKS
HCRM harnesses the power of the award-winning MITCH predictive modeling engine, a tool that creates a meticulous model of how health care ‘works’ for a specific population. With seamless integration of medical and pharmacy claims and EMR data, the model predicts clinical events and outcomes for each member of the population and attaches a dollar value to those predictions. AI is utilized to identify high-cost members based on historical data, and to predict future high-cost members based on their current clinical condition, health service utilization, care gaps, and likely outcomes. HCRM can create additional models specific to member populations with their MITCH model technology.
This tool, combined with our team’s training and support, is available both on-site and remotely accessible whenever and wherever you need it! We can also help you generate customized reports and assist in interpreting the results of our prediction processing, providing valuable insights into population health analytics.
HRM RISK MEASURES
The HCRM model accurately predicts cost by clinical risk drivers, pharmacy costs, emergency room visits, and inpatient days. The predictive modeling software provides measures of relative risk compared to national norms for each member. It also provides member risks for 1) being in an acute care environment (ER or inpatient care), 2) the long term impact of multiple co-morbid chronic conditions and poor compliance with standards of care, and 3) the member’s motivation to maintain or improve health behaviors.
